Used to gather additional current health information on campers (and potentially staff) including basic contact information, allergies, medications, and other pertinent medical information. The medical recommendation form should be completed by a licensed medical provider. The medical recommendation is accompanied by a four-page Camper Health History Form 1. These two forms will meet the ACA-accreditation standards for Health History and Health Exams (HW-2 and HW-6).
